SC construction stormwater forms: links to the agencies' own versions

Last checked Oct 20, 2026.

SWPPP HQ is an independent guide, not affiliated with SCDES or any county, city, or MS4. The forms below are published by the South Carolina Department of Environmental Services (SCDES, formerly SC DHEC) and by local MS4 programs. They are the agencies' forms, not ours. The current version is always the one on the agency's own website. Every download button opens the agency's own file. We don't host copies. This is general information, not legal or engineering advice. Forms and links can change, so check the agency's page before you file. If your site is inside a regulated MS4 (for example Greenville County or Anderson County), the MS4 may require its own forms as well.

Each listing shows the publisher, the file type and size, and the version printed on the form, where there is one. Sizes are the file size the agency's server reported on Oct 20, 2026. Citations like "CGP 4.2.F, p.54" mean section 4.2.F of permit SCR100000, on page 54 of the PDF.

1. Official SCDES forms you download and keep

These are filled out on paper (or in a PDF reader) and kept in the On-Site SWPPP. DES 0437 says contractor certifications "do not require Department approval"; each contractor gives the signed form to the owner/operator. Pre-construction conference attendance "must also be documented and maintained within the On-Site SWPPP (OS-SWPPP)" (DES 0436).

2. Forms filed online only, in SCDES ePermitting

SCDES says that after May 5, 2025, "all permit applications must be submitted exclusively through ePermitting." SCDES lists these forms as "Apply Online" and doesn't publish downloadable PDFs of them, so there is no download button here. Some MS4s post their own NOI forms for local review (section 4); check with your MS4.

3. No official SCDES form: what the permit requires instead

Weekly SWPPP inspection report: SCDES does not currently publish an official form. We found no inspection report form on SCDES's current construction stormwater, All Forms, or Design Aids pages (checked Oct 20, 2026). The SCDES SWPPP Template (section 1) has an Inspection Log, which lists when inspections happened. It isn't the report itself. The permit says that for each inspection "you must complete an inspection report" (CGP 4.2.F, pp.54–55) and lists what it must include, at a minimum:
  1. The inspection date
  2. Names and titles of the inspectors, and their qualifications if not given in an earlier report (unless they change)
  3. All discharge points
  4. Weather information and a description of any discharges at the time of the inspection
  5. Total rainfall since the last inspection
  6. Locations of discharges of sediment or other pollutants from the site
  7. Locations of BMPs that need maintenance
  8. Locations of BMPs that failed to operate as designed or proved inadequate
  9. Locations where additional BMPs are needed
  10. Corrective action required, including any changes to the OS-SWPPP and implementation dates
  11. Site name, operator name, and permit number
  12. Verification that all BMPs in the OS-SWPPP are installed and operating as designed
  13. Whether the construction sequence is being followed
  14. Status of corrective actions from the previous inspection, with the date each was addressed
  15. Items carried over from previous reports that were not addressed
Inspections are required at least once every calendar week, with no more than 9 days between inspections (CGP 4.2.B, p.53). The qualified inspector must sign each report, and reports are kept in the On-Site SWPPP for at least 3 years after coverage ends (CGP 4.2.H, p.55). Your MS4 may require its own form (see section 4).
Rain log: there is no stand-alone SCDES form. The official blank is the SWPPP Rainfall Records pages (pp.36–37) of the SCDES SWPPP Template (PDF, 683 KB). The permit "strongly recommend[s]" an on-site rain gauge, says rainfall amounts "must be maintained in a Rain Log located in the on-site SWPPP", and says rainfall for the day of the inspection and any significant rainfall since the last one must be reported on each weekly inspection report (CGP 4.2.D, p.54).
Small construction waivers (1–5 acres): no SCDES form. The permit says: "At the time of publication, a Low Erosivity Waiver Form is not available." The required contents of the waiver certification are listed in CGP Appendix B, pp.88–89.

4. Local (MS4) forms

Large MS4s run their own review and inspection programs. Large MS4s may require their own forms in their area; check with your MS4. We checked these six on Oct 20, 2026:

5. Unofficial blank forms (not SCDES forms)

These are not SCDES or MS4 forms. SWPPP HQ is an independent guide. We do not review, approve, or certify any SWPPP, site, or inspection, and we are not affiliated with SCDES or any MS4. Our templates are blank forms for the permittee's qualified inspector to complete; your SWPPP, permit, and MS4 control. Use them only if your SWPPP preparer and MS4 accept them.
